Welcome to Your 2016-2017 Benefits

 As a Quarles Petroleum associate you have choices in a comprehensive and valuable benefits program
 that you can match to your own personal situation. Whether you are electing benefits during the annual
 Open Enrollment period or as a newly hired associate, it’s important that you understand your options and
 take full advantage of your benefits to protect yourself and your family.

Enrollment Deadlines

To elect your benefits, you must enroll before the deadline.
  ? Newly eligible and newly hired associates must enroll w it hi n 30 days after becoming eligible for
       benefits. After you enroll, your elected benefits become effective the first day of the month following 30
       days of continuous employment.
  ? During Open Enrollment, which is usually held in April. Your elected benefits become effective
       on May 1 and continue until you change your elections.

If you miss the enrollment deadline, you may enroll during the next annual Open Enrollment period
or following a qualified change in status (see Changing Elections after You Enroll on page 3).
