Eligibility

You are eligible to participate in the benefits program if you are a regular full-time associate scheduled to
work at least 30 hours a week.

Dependent Eligibility

If you are eligible and enroll, you may enroll your legal spouse
and eligible children in the medical, dental, vision and optional
supplemental term life insurance. Your child or children are
eligible if they are your:

   ? Naturally born children
   ? Stepchildren
   ? Children who you or your legal spouse adopted or who

         are placed with you for adoption or foster care
   ? Children for whom legal guardianship has been

         awarded to you or your spouse
   ? Any children for whom you are required to provide

         coverage under a Qualified Medical Child Support
         Order (QMSCO) or other court or administrative order
   ? Children of any age if they are mentally or physically
         disabled and depend on you for financial support.*

   *You will need to provide proof of your child’s disability from
   a physician certifying that he or she was disabled before
   reaching the eligibility age limit and is incapable of self-
   sustaining employment.

When Coverage for Your Children Ends

Coverage for your dependent child(ren) continues until they reach age 26 for medical, dental, vision and life
insurance benefits.

A dependent child may remain covered on the medical plan after his/her 26th birthday if the child is incapable
of self-sustaining employment by reason of mental or physical handicap. Special approval must be obtained
from the insurance carriers.

When coverage ends, you may be able to enroll your children in COBRA continuation coverage. Contact the
Human Resources Department for more information.
