Contribution Rates

Associates and spouses have the same rates based on the age of the associate and the coverage amount
as shown in the table.

Example: Terry is age 42 and elects $50,000 in                       Supplemental Life

Disability Benefits

Short Term Disability Leave

Short Term Disability hours are accrued at the end of
each anniversary year and cap at 1,040 hours. Hours
are paid out at 100% of your pay rate and may only be
used with prior approval from HR.

Long Term Disability Plan

Quarles automatically enrolls you in the Long Term
Disability Plan at no cost to you if you regularly work at
least 30 hours a week.

If you are disabled following a qualifying accident or
illness, you can apply for Long Term disability benefit
equal to 60% of your base pay. Benefit payments
begin after six months of continuous disability and end
at age 65 or you are no longer disabled as defined by
the plan.
