Employee Assistance Program

During your lifetime, you may need extra support. Resource Advisor, through Anthem, is an Employee
Assistance Program (EAP) that offers you and your family members a range of personal services and
referrals at no cost to you. You can call Resource Advisor for help with many personal issues, including:

? Child care and parenting  ? Emotional well-being
? Helping aging parents     ? Addiction and recovery

? Financial issues          ? Wellness and prevention
? Legal concerns            ? Concierge and convenience services

? Work and career           ? Life events

To contact a Resource Advisor counselor, call 800-346-5484 24 hours a day, seven days a week. Your
discussions are completely confidential. You can also visit the website at anthemeap.com.

Education Assistance Program

Quarles believes in continuing education and we encourage associates to pursue opportunities to upgrade
their skills and complete degree programs. If you have been employed full-time by Quarles for at least 12
months, you may be reimbursed up to 80% of your tuition, registration, books and lab expenses to a
maximum of $5,250 per year. Payment will be made after you successfully complete each class that is
work-related or required for a business degree program.

Saving for a secure financial future is important and the Quarles 401(k) Retirement Plan through Fidelity
and the Quarles Profit Sharing Plan can help you reach your retirement goals.

                                                       401(k) Retirement Plan

                                                                      You may choose to contribute between 1% and 90%
                                                                      of your eligible earnings, up to the IRS annual limit.
                                                                      The limit for 2017 is the same as for 2016, $18,000.

                                                                      Federal law allows plan participants aged 50 or older
                                                                      and who make the maximum allowable contribution to
                                                                      make additional catch-up contributions. The maximum
                                                                      catch-up contribution is set by the IRS and may
                                                                      change from year-to-year. The maximum annual
                                                                      catchup contribution for 2016 and 2017 is the same,
                                                                      $6,000.

Matching Contributions
When you participate in the plan, Quarles will match 100% of the first 3% you contribute, and 50% of the
next 2% you contribute. That’s free money from the company to help you save for retirement.

Vesting (Ownership)
You are immediately 100% vested in your own contributions, Quarles’ matching contributions and any
investment earnings. You earn vesting in the Quarles’ profit sharing contributions at a rate of 20% a year
over five years.
