Enrolling in the 401(k) Plan
You are actively encouraged to enroll in the 401(k) Retirement Plan within 30 days of your hire date. After
30 days, if you do not actively enroll or opt out, you are enrolled automatically at a 5% contribution rate.
Your pre-tax contributions will be directed to a target date fund based on your date of birth.
To enroll immediately or to disable automatic enrollment:
? Go to Fidelity NetBenefits at www.401k.com.
? Create a password. If you are already a Fidelity customer, you can use your existing password. You
will be asked to provide your email address.
? Click on the link to enroll.
If you want to enroll in the 401(k) Plan before your first paycheck, contact Human Resources. If you have
any questions or need help before getting started, go to www.401k.com or call Fidelity at 800-835-5097.
Profit Sharing Plan
You are eligible to participate in the Profit Sharing Plan after six months of service if you have worked at
least 500 hours. Quarles makes 100% of the contributions. You become vested in the Profit Sharing Plan
over a five-year graded schedule. The Quarles contribution to the Profit Sharing Plan is discretionary and
not guaranteed.
Paid Time-Off (PTO)
Quarles recognizes that associates have diverse needs for time off from work and has established a paid
time off (PTO) policy to meet your needs. PTO promotes a flexible approach to time off. It can be used for
vacation, illness or disability, appointments,
